IF YOU THINK YOU MIGHT HAVE COVID-19, PLEASE STAY HOME. You should go the Emergency Department only if you are sick enough to need hospitalization, and if so, call ahead so they can prepare for your arrival.

Refills: When requesting refills, be sure to have the doctor's name, patient's name, date of birth, name of the medication, strength, frequency, and pharmacy information available. Please allow 3 business days for refills. You may request a refill by either calling or emailing:

Payment: Payment is due when services are rendered. If you have a balance, you will be asked to pay it before additional services are provided. Payment plans are available upon request. We accept credit cards, personal checks, and cash.
Missed Appointments: You will be charged a fee for missed appointments or appointments rescheduled with less than 24 hours' notice. Late fees must be paid to continue to obtain services. You may not be allowed to reschedule if you miss more than 3 appointments.
